What to Expect on Your First Visit

What to Bring

Please bring insurance cards and related medical information on your first visit.  We also recommend that you download and complete the PATIENT INTAKE FORMS prior to your visit so that you can take your time completing the information, without feeling rushed.

What to Wear

Please wear casual clothing. Clothing should be comfortable and allow you to move freely. If you forget the appropriate items, we have some to loan you.  Please wear some type of shoe that is safe and comfortable to exercise in.
